postgres_create_node

Tenant-global create-only node insertion, atomic across PostgreSQL writers.

A native insert runs in one transaction and checks the CORE-199 erase fence after its INSERT holds ROW EXCLUSIVE on memory_nodes, so a sweep that commits a job meanwhile either erases the row or makes this insert refuse and roll back. A v0 key can never equal a v1 job prefix, so v0 keeps its autocommit.
